Client: Hello?

 

PN: Hi. This is _____________. I am the Patient Navigator who will be taking you to your appointment on Friday.

 

Client: Oh, thank you so much. My appointment is at 10:30, so could you pick me up at 10:00?

 

PN: I’d like to give us a little extra time. Is 9:45 okay for a pick up time?

 

Client: Sure! I’ll see you then. Bye!

 

PN: Actually, I have a few questions that I’d like to ask you about the appointment so I can be helpful. Is now a good time?

 

Client: Yes

 

PN: Do you have a nickname or preferred name that you would like me to use?

 

Client: Actually yes. My name is Barbara, but I prefer to go by Barb.

 

PN: And is Barbara the name on your medical record?

 

Client: Yes, it is.

 

PN: Great! Is this a doctor you have seen before?

 

Client: It is.

 

PN: And how would you describe your relationship with her?

 

Client: I’ve only met her once before but she seems fine.

 

PN: What is the reason for this appointment?

 

Client: To follow up on a problem with my hip.

 

PN: What changes or symptoms do you want the doctor to know about?

 

Client: Well, I’m still in a lot of pain.

 

PN: And is there anything in particular you have been asked to bring with you?

 

Client: Yes. I have to bring the recent x-ray.

 

PN: And what is the pain preventing you from doing?

 

Client: I can’t walk for a long time and it really hurts when I climb stairs.

 

PN: Are there any changes in your life that the doctor should know about?

 

Client: I can’t really think of anything…

 

PN: What questions do you have for the doctor?

 

Client: I want to know if I can take a different medication for pain.

 

PN: Do you have any other questions…I will make a note of these.

 

Client: I wonder if I should have physical therapy?

 

PN: OK, I have written these down so we can be sure these get addressed. What other help would you like from me during this visit?

 

Client: I can’t think of anything…just want the doctor to get me feeling better!

 

PN: This must be frustrating for you! Hopefully you will get some answers at the appointment. I will plan to pick you up at 9:45 on Friday in front of your building. I have a black Toyota. I will call you earlier to remind you!

 

Client: Ok thanks!
